At least my own copy comes in Label's cover (jacket of ffrr) with the basic reference.

As stated above, the song 'Flesh' had a big influence on the Belgian dance scene, but only when it was discovered by a DJ that it plays better at 33rpm + 2% (or so) instead of the original 45 rpm. This started the shortlived "not faster than 120 bpm, manneke!" dance scene "New Beat".
