Most copies come with the 'clean' one-sided yellow & black sleeve as pictured. Some had stuff wrote on the sleeve by the band.
Some copies had a 'dirty' xerox sleeve (onto thin card as opposed to the thin paper the regular sleeve was printed on). See images.
Some sleeveless copies sold by Taang! came with either a 7"/7" piece of pink/red paper with 'The Sickness', etc, scrawled on it in black marker or a xerox of the regular sleeve on light green luminous paper.
