I have a version that was given to me by someone at Blast First Records in London in 1989 and has a mirror image of the cover. Also no inserts.

The best 'Butthole Surfers' Live album on double vinyl ! ! !

This LP is outstanding. The recording is very good and the concert itself must have been amazing to see live. It contains many of their early notable tracks as well as an REM cover ('The One I Love', Track B5) which is obviously a classic in its own right.

John E. Smoke live on Vinyl is one of the funniest songs i've enjoyed over the years in my collection. I love this double LP set. It also came out on CD but obviously since i only review vinyl this is the one i'd recommend.
