The first track "Suite: Judy Blues Eyes" is in fact just a short excerpt of the original song: it's equal to the intro of "4 Way Street".

Track 4.1 is actually a medley out of "You Can't Catch Me", a Chuck Berry Cover, and "Word Game", a Stephen Stills song.

The Song "Falling Down", the other unreleased Neil Young song here besides "Traces", might be better known as "Pushed It Over The Edge".

The songs are not credited on label or cover.

Some of the instruments heard remain uncredited.

I have the exact lineup of tunes, but the album is called We Waited 3 Years For This. "Great Live Concerts" is on each of the four labels on the record. No numbers other than 33 1/3.
All Rights Reserved. All Wrongs Reversed." Also claims to be stereo.
