This is the very first pressing of the CD version from 1995. It has a black band logo in the upper left corner and the title in red letters. Fold-out booklet with black and white photos inside. White letters on the back of tray card and on the spines - both spines are printed. The disk itself has a grey/silver print: MAYHEM and 666. Matrix is simply 666. Back of the tray card has a barcode because the CD was actually distributed by some smaller independent stores (those old geezers like me will surely remember how hard it was to find some albums before the internet days!). Later versions of this bootleg CD were bootlegged by someone else and have extra songs. They were not bootlegged by the same individual, just some money hungry bastards. I can't think of any other bootleg which was bootlegged so many times.....
