I mail ordered this thru Touch & Go when it first came out and it came with a little handbill inside the sleeve, the dimensions were 3 by 4 inches, or around that size. And the picture on the handbill was the NECROS name on top with the swords thru the skeleton image and on the bottom was a Ohio address, I can't be more specific because I sold my copy years ago, but I haven't seen this mentioned. And as an aside when I sold my pressing IQ 32, I slipped this handbill in that, later remembering that it went with the LP. So if you have this record check inside the sleeve, good luck.
