I could not locate my copy although the MO, Gatefold copy (Monarch) does come close. the labels are marked correctly but the runout is slightly different. Perhaps a variant?

The 3 etched on side one is circled as is the 2 on side two. The letter "A" before 24150 are oddly shaped as on side on it looks like a badly written "0" and o side 2 it look more like a clean triangle.
